What They're Looking For.
Must Have
3+ years post-residency work experience, 2+ years in-person primary care experience, Experience with virtual visits required, Active IMLC compact license, Prior experience with EPIC EMR, Clean background, Clean medical malpractice history, Board-certified in Family Medicine
Nice to Have
Spanish Speaking is a plus, Interest in developing longitudinal relationships, Comfort with management of acute conditions, Extensive experience handling 3+ cases per hour, Eagerness to learn about cutting-edge technology

What you'll be doing: Provide high-quality preventative, acute, and chronic care in a fully-remote, virtual setting Elevate remote care by providing compassionate and meaningful patient encounters Manage a dedicated panel of patients as their primary care physician Refer patients to specialists or acute care services when clinically indicated Work on population health initiatives for risk-attributed patients (AWVs, care gaps, transitions of care, risk capture) Support patients immediately post-discharge to improve understanding of treatment plans and reduce hospital readmissions Practice continuous self-evalua
