What They're Looking For.
Must Have
3+ years post-residency work experience, 2+ years in-person primary care experience, Experience with virtual visits required, Active IMLC compact license, Prior experience with EPIC EMR, Clean background, Clean medical malpractice history, Board-certified in Family Medicine
Nice to Have
Spanish Speaking is a plus, Interest in developing longitudinal relationships, Comfort with management of acute conditions, Extensive experience handling 3+ cases per hour, Eagerness to learn about cutting-edge technology
What You'll Do.
Provide high-quality preventative care
Provide compassionate patient encounters

Manage dedicated panel of patients
Refer patients to specialists
Refer patients to acute care
Work on population health initiatives
Support patients post-discharge
Improve understanding of treatment plans
Reduce hospital readmissions
