What They're Looking For.
Must Have
Bachelor's or Master's degree in social work, Licensed by the Minnesota Board of Social Work, Registered Nurse, Licensed by the Minnesota Board of Nursing, 2 years of work experience in the field of social services, Knowledge of health, social services, and funding sources, Proficient with technology, Demonstrated ability to manage multiple computer systems

working with older adults and/or people living with disabilities
What You'll Do.
Provide care coordination services
Coordinate services for individuals
Interact professionally with team members
Meet remotely and face-to-face
Integrate and improve coordination of care
Provide Care Coordination services
Advocate for individuals
Apply ethical practice principles
Promote collaborative relationships
Provide timely services
Manage a flexible caseload
Attend all required meetings
Practice customer service excellence
Demonstrate written communication
Complete documentation of services
Meet compliance standards
Respond and report all incidents
Meet completion deadlines for training
Comply with federal and state laws
Travel throughout St. Louis County
Perform other duties as assigned
