Positions at Spencer Hospital are not affiliated with Avera; however, we collaborate to provide services to the communities we serve. Employment offers, if accepted, are exclusively with Spencer Hospital. ** **Responsibilities:** * The Nurse Navigator serves as a consistent care coordinator, ensuring timely access and seamless coordination of services throughout the continuum of oncology care. * The Navigator will meet with patients and families to provide emotional support and guidance following the diagnosis of cancer through end state disease if necessary. * Provide medical information; education; and share knowledge regarding internal and external support services and resources. * Responsible for facilitating: rapid scheduling, coordination follow-up care and tracking patients to ensure completion of recommended treatment plan. * Identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources. * The Navigator will be an integral part of and professional member of the Interdisciplinary Team promoting optimal patient. * Serves as the tumor board coordinator; a responsibility that requires extensive preparation, facilitation during sessions and submission of detailed documentation to the accreditation panel following each meeting. **Job Requirements:** * Active IA RN license * Minimum 1 year oncology experience required * Oncology Nurse Certification preferred * Excellent communication, organization and time management * Must be a caring communicator, flexible, able to multitask and be comfortable with technological
