Must Have
Certified Medical Assistant certification, Registered Medical Assistant certification, 1+ years clinical setting experience, Proficiency in medical terminology, Proficiency in clinical processes, Highly organized, Detail-oriented, Capable of handling multiple tasks
Nice to Have
Experience in care management role, Experience in chronic disease management role, Working Spanish knowledge
What You'll Do.
Assist with Chronic Care Management programs
Assist with Remote Patient Monitoring programs
Engage with patients in person
Review patient care plans
Answer patient questions
Ensure patients on track
Serve as liaison between patients
Serve as liaison between clinical staff
Serve as liaison between care management team
Assist clinic staff with enrollments
Assist clinic staff with follow-ups
Assist clinic staff with care monitoring
Support physicians with patient care
Support clinical staff with patient care
Prepare health data for patient visits
Work closely with care management team
Streamline care delivery
Identify areas for process improvement
