What They're Looking For.
Must Have
Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification required, Bilingual English and Spanish, verbal and written, 1+ years of experience working in a clinical setting
Nice to Have
preferably in a care management or chronic disease management role
What You'll Do.
Work directly in clinics to assist with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs
Engage with patients in person to review their care plans
and ensure they are on track with their health goals
Serve as a liaison between patients
and Medsien’s care management team to ensure effective care coordination
Assist clinic staff with program enrollments
and ongoing care monitoring
Support physicians and clinical staff in providing seamless patient care by preparing relevant health data for patient visits
Work closely with Medsien's care management team to streamline care delivery and identify potential areas for process improvement
