Reports To: Executive Director Luminary Hospice is offering a 5k Sign-on Bonus! We are centered on caring for the whole person – with expert medical care, pain management as well as emotional and spiritual support along with resources, information, and emotional support for families and caregivers. About the role: The after-hours registered nurse supports the patient needs of the agency in the evening during the weekdays, working overnight shifts Monday through Thursday (5:00 PM – 8:30 AM) and a continuous weekend shift from Friday at 5:00 PM through Monday at 8:30 AM. This role follows a rotating schedule: one week on, followed by one full week off. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities after normal business hours. Job Responsibilities: Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process and utilizes health assessment data to determine nursing diagnosis. Assesses and evaluates patient's status through writing and initiating plan of care, re-evaluating patient and family/caregiver needs after hours when needed, and participates in revising the plan of care as necessary.
