What You'll Do.
Organize hospice care
Meet patient and family needs
Cover call for nights and weekends
Complete initial assessment
Complete comprehensive assessment
Complete ongoing assessment
Determine hospice needs
Provide physical assessment
Provide history of illness
Respond to calls from Triage
Make visits to patients
Complete weekend admissions
Utilize nursing process
Assess patient status
Evaluate patient status
Initiate plan of care
Re-evaluate patient needs
Re-evaluate family needs
Make necessary revisions
Use health assessment data
Determine nursing diagnosis
