On-Call, 12- or 24-hour weekend rotation may be required, 1099 contract opportunity, Care provided at patient’s residence, Background check required, Must remain reachable during scheduled on-call hours, Prepared to report to assigned patient homes during scheduled on-call hours, Failure to respond or report may result in forfeiture of on-call pay, Failure to respond or report may result in removal from the on-call roster, Prepared to work patient shifts of up to 12 hours when activated, Maintain required insurance, Ability to stand or walk for prolonged periods, Ability to lift up to 50 lbs, Ability to move patients, Ability to operate under stressful conditions, Must have dependable transportation, Willingness to commute to the patient’s home, Ability to provide basic medical management up to medically fragile end of life care, Ability to assist with two person assist

What They're Looking For.

Must Have

Associate or Baccalaureate School of Nursing, Current State or Compact License as a Registered Nurse or Licensed Practical Nurse, Current CPR certification, TB test and physical examination, Maintain required insurance, Good interpersonal skills, Hand-eye coordination and manual dexterity, Ability to stand or walk for prolonged periods, Ability to lift up to 50 lbs, Ability to move patients, Ability to operate under stressful conditions, Ability to make quick decisions, Dependable transportation, Willingness to commute to the patient’s home, Ability to provide basic medical management up to medically fragile end of life care

Nice to Have

Minimum of 1 year experience as an RN/LPN in a clinical or home health setting, Ability to assist with two person assist

What You'll Do.

Remain available during scheduled on-call hours

one-on-one patient care

Travel to assigned patient homes

Deliver care in accordance with care plans

Document care accurately

Communicate updates to the care team

Execute physician prescribed plans of care

Complete compliant documentation

Manage and administer prescribed medication

Conduct patient assessments

Perform activities associated with daily living

Monitor vitals and GI intake

Assess skin integrity

Administer wound care

Monitor for changes in patient condition

Communicate with family members

Communicate with physicians

Communicate with case managers

Provide education and training to patient

Provide education and training to family

Provide education and training to Home Health Aide

Work with patients one-on-one

Collaborate with CNAs

Supervise Certified Nursing Assistants

Collaborate with Personal Care Attendants

Supervise Personal Care Attendants

Perform other personal care services
