- Role: Occupational Therapist (OT) - Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) - Location: Field Based - Compensation (Productivity Points): $75.00 / point WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES 1. Carry a high-quality caseload 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Complete in-home OT evaluations and deliver skilled OT visits. - Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence. - Recommend and train on adaptive equipment and home modifications as needed. - Coordinate with nursing and the broader care team when condition or needs change. - Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent. WHO YOU ARE / HOW YOU OPERATE - Dedicated clinician: home health experience strongly preferred - High ownership: you treat your caseload like your own panel
