Collaboratively address the occupational needs of individuals, groups and populations via provision of quality and outcome oriented occupational therapy services, by assessing and interpreting individuals’ and group assessment and evaluation results and determining appropriate treatment. 2\. Develop a plan of care and group protocols to help individuals and groups address barriers to occupational participation via person and occupation-centered interventions. 3\. Lay out the types of activities and specific goals to be accomplished which will be incorporated in the individual’s person-centered plan 4\. Document goals, applicable interventions and progress adhering to reimbursement and practice act standards. 5\. Collaborate with treatment team for care coordination and incorporation of OT services in the individual’s person-centered plan. 6\.
