Must Have
Current MD Occupational Therapist license, 1 year recent OT experience, Graduation from accredited OT program
Nice to Have
Prior home care experience preferred, Computer prior experience with EMR preferred
What You'll Do.
Perform thorough evaluations of patients’ functional status
psychological and motor aspects of performance
Develop individualized intervention plans
Evaluate home environment for hazards
Identify equipment needs
Identify environmental adaptations
Continually assess and revise the occupational therapy care
Participate with nursing in the multidisciplinary care plan
Educate and instruct patients
Document observations
