Current Maryland Occupational Therapist license, 1 year recent work experience as Occupational Therapist, Graduation from accredited occupational therapy program
Nice to Have
Prior home care experience preferred, Solid computer prior experience with EMR preferred
What You'll Do.
Make home visits to clients
Perform thorough evaluations of patients’ functional status
psychological and motor aspects of performance
Develop individualized intervention plans
Evaluate home environment for hazards
Identify equipment needs
Identify environmental adaptations
Continually assess and revise the occupational therapy care
Participate with nursing in the multidisciplinary care plan
Educate and instruct patients
Document observations
Document patient care management
Document services provided
