Eligibility Requirements

Ability to travel to participant homes and clinical sites within the designated service areas, Must be medically cleared for communicable diseases and have all required immunizations prior to direct participant contact, Must meet all regulatory requirements applicable to the PACE program, Physical Requirements: Standing, Sitting/Stationary Positions, Lifting/Push/Pull, Bending/Squatting, Stairs/Steps/Walking/Climbing, Agility/Fine Motor Skills, Sight/Visual Requirements, Audio Hearing and Motor Skills (Language) Requirements, Cognitive Ability

What They're Looking For.

Must Have

Bachelor of Science in Nursing (BSN) from an accredited nursing program, Current, active, and unrestricted Registered Nurse (RN) license in New York State or eligibility for licensure, Ability to travel to participant homes and clinical sites within the designated service areas, Proficiency with computers and electronic health record systems, Must be medically cleared for communicable diseases and have all required immunizations prior to direct participant contact, Must meet all regulatory requirements applicable to the PACE program, Ability to work collaboratively within an interdisciplinary team, Demonstrated compassion and commitment to patient-centered care, Willingness to learn and develop clinical expertise, Ability to sustain standing for up to 6 hours a day, Ability to sustain sitting/stationary positions for up to 6-8 hours a day, Ability to lift up to 50 pounds, Ability to safely lift patients using OSHA guidelines, Ability to safely bend or squat to care for patients, use medical supplies, Ability to safely maneuver stairs, climb up/down, and walk to access work areas, Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools, Must be able to visually assess patients, read orders, type/write documentation, etc., with accuracy, Must be able to listen attentively, document information from patients, community members, providers, etc., and accurately process intake information through audio processing, Must be able to speak comfortably and clearly with language and motor skills so that customers can understand the individual, Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job

Nice to Have

Previous clinical rotations or internships in geriatric, community health, home care, or long-term care settings, Demonstrated interest in working with frail or elderly populations

What You'll Do.

Deliver nursing care to participants in accordance with individualized care plans

and evidence-based nursing practice standards

Conduct supervised home visits

and document participant health conditions

Report changes to the interdisciplinary care team and supervising nurse

Support disease management and health promotion for participants with chronic conditions

Maintain accurate and timely documentation in the organization's electronic health record system

Adhere to all clinical protocols

regulatory requirements

and infection control standards

Meet the competency standards established by the PACE organization prior to independent practice

How You'll Work.

Team & Collaboration

Interdisciplinary collaboration; Work closely with physicians, nurse practitioners, social workers, rehabilitation therapists, and other interdisciplinary team members; Support care coordination efforts to ensure participants receive comprehensive, high-quality care; Participate in care planning discussions and team-based decision making
