What They're Looking For.
Must Have
Valid current RN license from the state or from a compact state, where applicable, CPR and de-escalation and restraint certification required
Nice to Have
One or more years’ nursing experience within clinical area of service provided by facility, Experience in behavioral health/psychiatric/addiction recovery environment, Certified in Psychiatric Nursing, First aid may be required based on state or facility requirements
What You'll Do.
Providing nursing care to clients
Supervising care treatment and services of client care provided by LPNs
CNAs and other staff on assigned unit and shift
and evaluating an individual plan of care for each client
Assessment and re-assessment of clients
Communicating appropriate information with the interdisciplinary team
Reporting client concerns and changes in condition to supervisor and/or provider in a timely manner
Coordinating care by assessing physical and behavioral health needs of client
Developing and implementing nursing care plans
Maintaining medical records
Educating clients and their families about various physical and behavioral health conditions
Ensuring client safety at all times
Assessing clients’ status on an ongoing basis
Communicating findings to the providers timely
Ensuring nursing treatment plans are specific to assessed needs
Completing initial treatment plan upon admission
Obtaining provider’s orders (verbal or via telephone) accurately
Documenting transcribing and reviewing provider’s orders ensuring all required elements are present and appropriate per policy and procedures
Accurately administering medications
utilizing at least two client identifiers
Notifying providers of significant changes in status such as difficulties with detox
pertinent lab results
or change in medical status regarding response to medications
Assessing and documenting effects of PRN medications
Maintaining accountability of administration
control and security of medications
Completing admission and discharge processes efficiently and accurately as directed
Confers regularly with treatment team (e. g.
provider) regarding client care
Attending and participating in the unit activities such as therapy groups
family meetings as appropriate
Providing education/explanation to clients
and co-workers regarding medication usage/maintenance
as well as other topics necessary for optimal client care
Monitoring client acuity
Escalating coverage needs to supervisor/Charge RN
accurate documentation is completed for all clients on each shift
Completing client assignment sheets daily
Carrying out tasks as assigned
Communicating thorough hand-off reports to oncoming staff and supervisors
