Master's degree in Nursing, Current RN (EC) registration, Certification as a nephrology Nurse (C. Neph) or commitment to complete, Minimum 2 years experience in care of patients with CKD, Minimum 2 years NP experience

What You'll Do.
Formulate clinical decisions for patients with CKD
Contribute to care plans for patients with CKD
Liaise with Nephrologists and primary care
Ensure seamless transitions home
Manage nephrology consults
Monitor renal patient needs
Manage renal patients in chronic service
Round in hemodialysis pods
Address immediate health needs
Liaise with renal program members
Use legislated authority to diagnose
Order diagnostic tests
Interpret diagnostic tests
Prescribe pharmaceuticals
Perform specific procedures
Provide direct patient care
Support patients with CKD
Identify clinical practice issues
Manage acute/chronic illness
Address educational needs

Working as a member of the Renal Program, the Nurse Practitioner (NP) will report to the Clinical Director. Working as a Registered Nurse in the Extended Class (RN (EC)), the NP will formulate clinical decisions and contribute to care plans for patients with chronic kidney disease (CKD). The incumbent will be the key liaison with Nephrologists, primary care, referral partners and the community to ensure seamless transitions home and to Nephrology ambulatory clinics. This role will rotate between the inpatient and outpatient clinical areas. The inpatient NP work will focus on responding to and managing inpatients and working collaboratively with the acute Nephrologist on call
