M-F Traditional Work Hours, No Call, No Weekends, Work from home 1.5 days a week, Comfort with End-of-life care discussions imperative, Comply with all HIPAA regulations, Maintain security of protected health information (PHI), Valid California driver license, Current automobile insurance, Physical demands: regularly required to talk or hear, stand, walk, sit, use hand to finger, handle or feel objects, tools, or and reach with hands and arms, Frequently lifts and/or moves up to 10 pounds, Specific vision abilities: close vision and the ability to adjust focus
What They're Looking For.
Must Have
Active California state license as a Nurse Practitioner/Physician Assistant, Active Nurse Practitioner/Physician Assistant Board Certification, Medicare Part D Prescriber, Valid California driver license and current automobile insurance, Active RN License, Active NP License, Furnishing number, NPI Number, DEA Number, Board Certification: AANP, ANCC, Valid BLS (Basic Life Support), Active PA License, Board Certification: NCCPA, Computer literacyR proficiency, At least one (1) year prior clinical experience
Nice to Have
Valid ACLS (Advanced Cardiovascular Life Support), Valid ACLS (Advanced Cardiovascular Life Support), Knowledge of current clinical standards of care, Ability to work independently, Experience in care of older adult (geriatric) patients, Home care experience, Excellent administrative, organizational, and verbal skills, Effective communication skills especially with older adults, Previous EPIC experience
What You'll Do.
Conduct in-home assessments
Outpatient clinical work
Obtain comprehensive history
Perform physical exam
Conduct medication review
Perform appropriate cognitive/depression/safety screenings
Collect laboratory specimens in the home setting
Identify diagnoses for active medical management
Communicate findings to PCP
Provide patient/family/caretaker education
Monitor patient needs
Follow up on patient needs
Refer to community resources
How You'll Work.
Team & Collaboration
Work closely with physicians, advanced practice providers, social workers, case managers, nutritionists, and other members of our health care team; Participate in multidisciplinary conferences
Communication Scope
Excellent verbal skills; Effective communication skills especially with older adults
