Full Job Description

## The Role Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. ## What You’ll Do Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers. Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows. Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline. Close the loop: Complete documentation/charts within the same day High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts. Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
