Full Job Description

## The Role Transform home-based care with AI. As Circadia’s home health NP, you’ll see patients first in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions—reducing preventable hospitalizations. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting. ## What You’ll Do Direct patient care: Conduct initial and follow-up in-home assessments on flagged patients; initiate/adjust plans of care; complete consent; coordinate with referring and primary care providers. Tech → bedside integration:Train and coach caregivers and home health aides at the point of care, standardize use of Circadia into home health clinical workflows Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits. Close the loop: Complete documentation/charts within the same day Home-based visit management: Efficiently complete 6–8 in-home visits per day across designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts. Quality & product feedback: Surface field insights to improve workflows and tech integration in a home health setting Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams. Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy. ## How Success is Measured (KPIs) Visits: Initially targeting around 6 days per month, with flexibility to increase over time (10+ days) Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
