Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant, Current State License as a Nurse Practitioner or Physician Assistant, Sit, stand, bend, lift, and move intermittently, Must be able to lift at least 35lbs and bear the weight of an average adult effectively, Current and valid driver’s license, Reliable transportation and valid automobile insurance coverage

Nice to Have

Post-acute or Nurse Practitioner/Physician Assistant experience preferred, Adult, Family, or Gerontology certification preferred, Experience with geriatric patients preferred

What You'll Do.

Provide Primary Care and/or Consultative Professional Care in a variety of care settings.

Function in collaboration with Physicians according to a Collaboration Arrangement.

Focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.

Conduct medical evaluations of predominantly elderly

seriously ill patients in their home environment.

Determine follow-up assessments according to patient needs and best practice guidelines.

Communicate significant findings

and changes in condition or environment to the clinical coordinator

and other personnel involved with patient care.

Assess health status by obtaining relevant health and medical history.

Perform a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician.

and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and healthcare needs.

Perform diagnostic or therapeutic interventions consistent with certification

and state medical privileges.

Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.

Develop a treatment plan in collaboration with the patient and caregivers.

Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment.

Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.

Provide patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.

Hold case conferences with families when appropriate to advance the care plan.

Meet minimum productivity requirements based on practice standards.
