Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant, Current State License as a Nurse Practitioner or Physician Assistant, Current and valid driver's license, Reliable transportation and valid automobile insurance coverage, Ability to handle multiple priorities, documentation requirements, and deadlines

Nice to Have

What You'll Do.

Conduct medical evaluations of predominantly elderly

seriously ill patients

Determine follow-up assessments according to patient needs

Communicate significant findings to clinical coordinator

Assess health status by obtaining relevant health and medical history

Perform a physical exam based on age and patient history

and evaluate laboratory tests

Perform diagnostic or therapeutic interventions

Prescribe and monitor medications

Develop a treatment plan in collaboration with patient and caregivers

Refer patients to physicians as appropriate

Ensure continuity of care by serving as a liaison

Provide patients and families an understanding of diagnosis

Hold case conferences with families when appropriate

Meet minimum productivity requirements
