What They're Looking For.
Must Have
Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant, Current State License as a Nurse Practitioner or Physician Assistant, Current and valid driver's license, Reliable transportation and valid automobile insurance coverage, Ability to handle multiple priorities, documentation requirements, and deadlines
Nice to Have
Post-acute or Nurse Practitioner/Physician Assistant experience, Adult, Family, or Gerontology certification, Experience with geriatric patients
What You'll Do.
Conduct medical evaluations of predominantly elderly
seriously ill patients
Determine follow-up assessments according to patient needs
Communicate significant findings to clinical coordinator
Assess health status by obtaining relevant health and medical history
Perform a physical exam based on age and patient history
and evaluate laboratory tests
Perform diagnostic or therapeutic interventions
Prescribe and monitor medications
Develop a treatment plan in collaboration with patient and caregivers
Refer patients to physicians as appropriate
Ensure continuity of care by serving as a liaison
Provide patients and families an understanding of diagnosis
Hold case conferences with families when appropriate

Function in collaboration with Physicians; Serve as a liaison between patient and other members of the multidisciplinary care team; Collaborate with colleagues to advance population health initiatives; Engage in weekly touchpoints with peers and management
