What They're Looking For.
Must Have
Active NP license in good standing, Board certification through ANCC or AANP as a Family, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care NP, Minimum 2 years of post-licensure clinical experience in primary care, internal medicine, endocrinology, cardiology, or geriatrics, Demonstrated comfort with telehealth or digital care delivery, documentation discipline, familiarity with EHR systems, Excellent independent clinical judgment, US residency
Nice to Have
IMLC/eNLC compact license strongly preferred, Experience with value-based care, CMS quality measures, or ACO/care management programs, Familiarity with CKM (Cardiovascular-Kidney-Metabolic) conditions and GLP-1 therapy management, Comfort prescribing under Collaborative Practice Agreements in non-Full Practice Authority states, Spanish language proficiency, Prior experience building or launching a new clinical program or service line
What You'll Do.
Conduct virtual medical evaluations
Create and manage holistic care plans
Act as primary prescriber
Manage device-triggered Red Alerts
Provide warm handoffs to clinicians
Document all patient interactions
Capture comorbidities accurately
Prepare PCP care coordination reports
Develop and refine titration algorithms
Contribute to building scalable workflows
Provide feedback on app-based care tools
