Must Have
Master's degree in Nursing, 2+ years Family Nurse Practitioner experience, 2+ years Primary Care Nurse Practitioner experience, 2+ years cardiology experience, 2+ years Nephrology experience, 2+ years EHR experience, Current state specific licensure, National Board Certification, Current DEA license, Current BLS or CPR Certification, Efficient transportation, Active driver's license, Basic computer skills, Microsoft Office skills, Internet Connectivity

CKD management expertise, ESRD care expertise, Dialysis management expertise, Experience leading care transitions, Patient education experience, 2+ years EHR experience, Wound Care certification, Experience using audio-visual technology
What You'll Do.
