What You'll Do.
Complete thorough patient intakes
Complete patient screenings
Complete patient assessments
Incorporate patient needs into plan of care
Incorporate family needs into plan of care
Triage referrals for acute needs
Collaborate with interdisciplinary team members
Coordinate compendium of care for each patient
Utilize nursing process
Utilize scientific reasoning
Utilize critical problem solving
Practice within scope of responsibilities
Place orders in accordance with protocol
Place screening tests in accordance with protocol
Provide appropriate clinical documentation
Communicate effectively with patients
Communicate effectively with families
Communicate effectively with clinical teams
Participate in annual performance reviews
Welcome new families into model
Share overall model with families
Help families access services
Gather information about neurodevelopmental challenges
Conduct telehealth-based diagnostic assessments
