What They're Looking For.
Must Have
Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification, Active, unrestricted NP or PA license in the state of practice, Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions, Computer proficiency (EMR, email, etc.), Tuberculosis (TB) screening program participation
Nice to Have
Experience in Medicare Advantage settings, Familiarity with HEDIS quality indicators and Medicare guidelines

Provide comprehensive care to seniors
Collaborate with care team
Coordinate hospital/SNF services
Manage home health services
Participate in daily huddles
Participate in on-call rotation
Support growth initiatives
