As a **Nurse Practitioner (NP) or Physician Assistant (PA)** at **Centerwell Senior Primary Care** , you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. You'll be the provider of the day seeing walk-in and some hospital follow-up patients and referrals. # **Use your skills to make an impact** What we're looking for: Required * Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification. * Active, unrestricted NP or PA license in the state of Florida * Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions * Proficiency using an EMR * If selected for this role, you will be required to be screened for TB. * Fluent
