What They're Looking For.
Must Have
MS in Nursing, Active, national NP certification, Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Arizona, Kansas, Iowa, Indiana, Ohio, and Florida, Valid, current, active, RN Compact license, 2+ years of experience in managing complex care cases, Experience working with Electronic Medical Records (EMR)
Nice to Have
Possession of DEA registration or eligibility, Active Medicaid number in the state of Arizona, Kansas, Iowa, Indiana, Ohio, and Florida, Experience in managing complex care cases for developmental disabilities and chronically ill patients, Experience working in a hospice environment
What You'll Do.
Provide primary and urgent health care via telephone and tele video
Develop and implement clinical plans of care
Gather history and physical exam
Identify and close gaps in care
Meet patient’s and family’s physical and psychosocial needs
Educate patients and families
Maintain contact with other clinical team members
Maintain patient medical records
Participate in continuing education
