What They're Looking For.
Must Have
MS in Nursing, Active national NP certification, Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Ohio, Valid, current, active, RN Compact license, Experience working with Electronic Medical Records (EMR), 2 + years of experience in managing complex care cases
Nice to Have
Active Medicaid number in the state of Ohio, Possession of DEA registration or eligibility, Experience in managing complex care cases for developmental disabilities and chronically ill patients
What You'll Do.
Provide primary and urgent health care via telephone and tele video
Develop and implement clinical plans of care
Gather history and physical exam and diagnostics
Identify and close gaps in care
Meet patient’s and family’s physical and psychosocial needs
Educate patients and families about medication usage
Maintain contact with other clinical team members
Maintain patient medical records and medical documentation
Participate in continuing education

Collaborate with company physicians; Collaborate with patient’s other physicians and providers; Collaborate with family members; Work with the company’s inter-disciplinary team; Maintain contact with patients’ other physicians and patients’ other medical providers
