We are so glad you are interested in joining Sutter Health! **Organization:** SMF-Sutter Med Foundation- Central **Position Overview:** The Nurse Navigator is a professional registered nurse (RN) who provides individualized support to patients, families, and caregivers to help overcome barriers within the healthcare system. This role is instrumental in enhancing the patient experience and improving clinical outcomes by facilitating a seamless navigation process across the continuum of care. The RN is responsible for collaborating with physicians and interdisciplinary teams to triage, coordinate, and manage patient care consistently. They serve as an essential point of contact for patients and families, offering education, advocacy, and emotional support. The Nurse Navigator participates in strategic initiatives within the clinical program or service line, including the development, implementation, and evaluation of navigation activities for long-term sustainability. This role fosters shared decision-making and patient engagement by forming trusting relationships, addressing communication and health literacy needs, and supporting patients in incorporating their values and preferences into treatment decisions.
