You will own the triage function end to end: reviewing referrals and clinical documentation, conducting structured telephone and video assessments, and determining the right care pathway for each patient. This role does not include the delivery of treatment, but it is not a passive one. Our patients have often exhausted conventional pathways and present with complex, multi-layered needs.
