Industry & Context.
No scheduled evening/weekend responsibilities outside of phone On-Call. Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8. 1. 24). 24/7/365 physician back up. Providers cannot receive care in clinic and provider’s household members cannot receive primary care in the clinic.
What They're Looking For.
Must Have
At least 4 years of clinical experience in an ambulatory care setting. You have active, unrestricted licenses to practice in 5+ states, and a willingness to obtain additional as needed by the organization. You are authorized to work in the United States. Must have access to high-speed internet and phone lines to support a fully remote practice.
Nice to Have
Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, with 10+ state licensure and willingness to obtain additional as needed. Alternatively - Master's degree in Physician Assistant studies certification by the Physician Assistant certifying body (NCCPA), with 10+ state licensure and willingness to obtain additional as needed. Family or internal experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. Preferred states include CA, TX, FL, NY, PA, IL, OH, GA, NC, MI. You’re dedicated to staying current with the latest developments in primary care and obesity medicine. You believe in inclusive, compassionate care. You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals. You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame. You’re willing and excited to try new technologies that are both patient and provider oriented.
What You'll Do.
Provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a virtual primary care setting.
Practice exceptional medicine within knownwell - Sees adults for metabolic health services as is appropriate in a virtual setting.
Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
How You'll Work.
Team & Collaboration
Coordinates with the broader care team to ensure the patient has the support they need. Escalate any concerns to your dedicated site leadership team. Provide clinical guidance to medical assistants and nursing teams as assigned. Excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff.

As a Fully Remote Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a virtual primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health. Schedule: Monday-Friday with flexible scheduling (32 patient contact hours minimum + 8hrs of administrative time). Fully Remote.
