Must Have
High School Diploma or equivalent, 1 year of relevant experience Pharmacy Required, LIC-Pharmacist Intern - STATE_MI State of Michigan Upon Hire required, LIC-Pharmacy Technician - STATE_MI State of Michigan Upon Hire required, CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required, CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required
Nice to Have
Associate's Degree health care or related field, 2 years of relevant experience Retail Pharmacy Preferred
What You'll Do.
obtain the best possible medication history for each patient admitted to the hospital
claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment
follow a structured interview with the patient or caregiver and document the information into the electronic medical record
communicate discrepancies directly to the provider/pharmacist for resolution
Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist
and interprets physician/nurse medication histories
under the direction of the pharmacist
and completes full history documentation in computerized system
Clarifies information through communication with providers
Performs special projects for the department
Works with multiple medication automation systems
Provides professional customer service as a liaison between the pharmacy department and patients/caregivers
Interacts with prescriber as needed
Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles
Reviews profile for allergies
and last doses and brings these to the attention of the pharmacist
Checks and verifies accuracy of patient profiles
Maintains clean and organized work area
Maintains all requirements for CPhT certifications
Completes all Corewell Health assigned competencies by assigned due dates
Perform quality assurance data collection
