In this role, you will be the critical link between primary and specialty care practices and our clinical teams, ensuring that the right patient information reaches the right people at the right time. What you'll do As the Medical Records Coordinator, you will own the end-to-end process of obtaining, organizing, and transferring clinical records from hospitals, physicians, and other healthcare providers into the Diverge Health Electronic Medical Record (EMR), supporting our patient-facing teams in delivering timely, high-quality care in securing government benefits for our most complex patients. Key areas you'll add value Obtain provider and practice information through patient interviews and requesting their medical records directly from hospitals, physician practices, and other healthcare providers or locating them directly within theEMR systems of Diverge Health provider network partners Experience with organizing complete patient medical histories, including all primary care, specialist, behavioral health, labs and visit notes from all relevant providers Complete assessments, tracking progress and attaching and uploading clinical documentation obtained from the above sources into the Diverge Health EMR, ensuring records are accurate, complete, and accessible fo
