What You'll Do.

Develop service plans

Monitor patient progress

Advocate on behalf of patients

Comply with data entry requirements

Conduct Needs Assessment/Intake

Document needs assessment data

Provide case management services

Educate clients on services

Facilitate case management groups

Participate in rounds

Maintain HIPAA standards

Perform documentation in electronic chart

Participate in QI initiatives

Develop operational protocols

How You'll Work.

Team & Collaboration

Collaborate with front desk/administrative staff; Serve as a key member of the IRHC Case Management Team; Actively participate in case management model development; Provide consultation to clinicians; Facilitates interdisciplinary consultation; Participates in multidisciplinary/staff/team meetings; Meets regularly with leaders

Communication Scope

Effective verbal and written communication skills in English; Demonstrated ability to write contract reports that are clear and informative; Effective interpersonal skills
