Assess patients' psychosocial status

Participate in care planning
Identify effective interventions

Utilize FKC patient education programs
Meet patient treatment goals
Monitor patient adjustment to dialysis
Provide supportive counseling
Provide educational counseling
Provide goal directed counseling
Assist with referral to community resources
Facilitate optimal treatment outcomes
Maintain current knowledge
Assist patients with referral
Assess patient awareness of advance directives
Assist with accessing advance directive forms
Facilitate discussion of advance directive wishes
Participate in discussion of patient DNR status
Ensure patient/family understand DNR status
Make informed decision about care
