Promote positive adjustment, rehabilitation and improved quality of life for our patients; Ensure comprehensive quality care of our patients; Enhance the likelihood that patients will achieve the FKC Quality Goals; Help the patient meet rehabilitation, treatment goals, and improve quality of life; Address barriers and meet patient treatment goals; Adjust to dialysis and achieve optimal psychosocial status and quality of life; Facilitate optimal treatment outcomes; Enable them to remain employed, become employed or receive education; Ensure patient and/or family understand and make an informed decision about their care; Address non-adherence, quality outcome, and quality life concerns; Reports on quality indicators related to adherence; Address barriers that may affect adherence to treatment; Maximize adherence and adjustment

What They're Looking For.
Must Have
Masters in Social Work, State required license, Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
What You'll Do.
Provides psychosocial services to patients
Works with the health care team
educates and supports staff
Supports FKC commitment to Quality Indicators
Assesses patients' psychosocial status
Participates in care planning
Utilizes FKC patient education programs
Provides monitoring and interventions
Provides supportive counseling services
Provides educational and goal directed counseling
Provides information and assists with referral
Maintains current knowledge regarding vocational/educational rehabilitation
Assesses patient awareness of advance directives
Participates in the discussion of patient DNR status
Adheres to FMCNA Social Work Policy
Documents based on MSW interaction
Provides psychosocial support
Participates in monthly Quality review meetings
Assesses patient knowledge of kidney disease
Provides appropriate information about all treatment modalities
Facilitates the transplant referral process
Provides ongoing education to patient/family
Reviews patient rights and responsibilities
Collaborates with the team on QAI activities
Reviews Patient Rights and Responsibilities
Understands the referral and admission process
Interviews the patient to identify root causes
Collaborates and functions as a liaison
Provides information and education to patients
Collaborates with the Insurance Coordinator
Refers patients to patient billing solutions
Assists with interview process
Works with administrative support staff
Provides training to staff
Contributes and participates with weekly team huddles
Adheres to work defined caseload guidelines
Performs other related duties as assigned
