What They're Looking For.
Must Have
Masters in Social Work, State required license, Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed

What You'll Do.
Provides psychosocial services to patients
Works with the health care team
educates and supports staff
Supports the Fresenius Kidney Care (FKC) commitment
Assesses patients' psychosocial status
Participates in care planning
Provides monitoring and interventions
Provides supportive counseling services
Provides educational and goal directed counseling
Provides information and assists with referral
Maintains current knowledge regarding programs
Assesses patient awareness of advance directives
Participates in the discussion of patient DNR status
Documents based on MSW interaction
Provides psychosocial support
Assesses patient knowledge of kidney disease
Provides appropriate information about treatment modalities
Facilitates the transplant referral process
Provides ongoing education to patient/family
Reviews patient rights and responsibilities
Understands the referral and admission process
Interviews the patient to identify root causes
Collaborates and functions as a liaison
Provides information and education to patients
Collaborates with the Insurance Coordinator
Refers patients to patient billing solutions
Assists with interview process
Works with the administrative support staff
Provides training to staff
Contributes and participates with weekly team huddles
Adheres to work defined caseload guidelines
Performs other related duties as assigned

Team & Collaboration
Works with the health care team; In collaboration with the interdisciplinary team; As a member of the interdisciplinary team; Participates in care planning in collaboration with the patient and healthcare team; In collaboration with the physician and nurse; Participates in monthly Quality review meetings with the interdisciplinary team; With other members of the interdisciplinary team; Collaborates with interdisciplinary team on transplant waitlist management; Collaborates with the team on appropriate QAI activities; Collaborates and functions as a liaison for patient with Insurance Coordinators; In collaboration with Insurance Coordinators; Collaborates with the Insurance Coordinator; Contributes and participates with weekly team huddles
