What They're Looking For.
Must Have
Masters in Social Work, state required license, 2 – 5 years’ related experience
What You'll Do.
Provide psychosocial services to patients
Assess patients' psychosocial status
Participate in care planning
Utilize patient education programs
Provide monitoring and interventions
Provide supportive counseling services
Provide educational and goal directed counseling
Provide information and assist with referral
Maintain current knowledge regarding programs
Assess patient awareness of advance directives
Participate in discussion of patient DNR status
Adhere to FMCNA Social Work Policy
Documents based on MSW interaction
Provides psychosocial support
Participates in monthly Quality review meetings
Assesses patient knowledge of kidney disease
Provides appropriate information about modalities
Facilitates the transplant referral process
Provides ongoing education to patient/family
Reviews patient rights and responsibilities
Collaborates with the team on QAI activities
Reviews Patient Rights and Responsibilities
Understands the referral and admission process
Interviews patient to identify root causes
Collaborates and functions as a liaison
Provides information and education to patients
Collaborates with the Insurance Coordinator
Refers patients to patient billing solutions
Assists with interview process
Works with administrative support staff
Provides training to staff
Contributes and participates with weekly team huddles
Adheres to work defined caseload guidelines
Performs other related duties
