Masters in Social Work, state required license, Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
What You'll Do.
Provides psychosocial services
Works with health care team
Supports FKC commitment
Assesses patients' psychosocial status
Participates in care planning
Utilizes FKC patient education programs
Provides monitoring and interventions
Provides supportive counseling services
Provides educational and goal directed counseling
Provides information and assists with referral
Maintains current knowledge
Assesses patient awareness
Participates in discussion of patient DNR status
Documents based on MSW interaction
Provides psychosocial support
Participates in monthly Quality review meetings
Assesses patient knowledge
Provides appropriate information
Facilitates the transplant referral process
Provides ongoing education
Reviews patient rights
Understands the referral and admission process
Interviews the patient
Collaborates and functions as a liaison
Provides information and education
Collaborates with the Insurance Coordinator
Refers patients to patient billing solutions
Assists with interview process
Works with administrative support staff
Provides training to staff
Contributes and participates with weekly team huddles
Adheres to work defined caseload guidelines
Performs other related duties
