Must Have
Masters in Social Work, State required license, 2 – 5 years’ related experience
Nice to Have
Epic certification preferred
What You'll Do.
Assess patients' psychosocial status
Participate in care planning
Provide monitoring and interventions
Provide supportive counseling services
Provide educational and goal directed counseling
Provide information and assist with referral
Maintain current knowledge regarding programs
Assess patient awareness of advance directives
Participate in discussion of patient DNR status
Adhere to FMCNA Social Work Policy
Document based on MSW interaction
Provide psychosocial support
Participate in monthly Quality review meetings
Assess patient knowledge of kidney disease
Provide appropriate information about treatment modalities
Facilitate the transplant referral process
Provide ongoing education to patient/family
Review patient rights and responsibilities
Understand the referral and admission process
Interview the patient to identify root causes
Collaborate and function as a liaison
Provide information and education to patients
Collaborate with the Insurance Coordinator
Refer patients to patient billing solutions
Assist with interview process
Work with administrative support staff
Provide training to staff
Contribute and participate with weekly team huddles
Discuss any urgent patient issues
Adhere to work defined caseload guidelines
Perform other related duties as assigned
