The Brief

What You'll Achieve.

Ownership of territory P&L; Improve profitability; Plan effective strategies for financial well-being of Patient Pathways; Meet face to face with hospital partners to discuss current discharge processes; Structure, introduce and train hospital partners on the Patient Pathway program; Identify areas of improvement in the discharge process in individual local markets; Conduct regular update check-ins with hospital partners and Patient Pathways management before, during, and after implementation; Analyze, develop, and implement new ways for the company to differentiate itself in the marketplace; Develop in-depth knowledge of the company's service offerings, markets, current market share within individual markets, market competition, and strategies to secure new business; Work cross-functionally with DaVita and Non-DaVita operations leadership across chronic and hospital services teams to introduce Patient Pathways; Update local operations leadership on the progress and impact of the Patient Pathways program; Discuss ongoing strategies for each market with Patient Pathways and field management; Provide coverage assistance at Patient Pathways partner hospitals as needed (including but not limited to covering during teammate PTO leave or position vacancies); Analyze Patient Pathways reports for assigned territory including but not limited to patient volume for each Dialysis Coordinator and length of stay and readmissions metrics for territory and individual hospitals; Analyze performance of each market and make recommendations on future action plans based on the analysis; Provide management team with monthly updates regarding the above metrics; Experience driving results for your book of business; Mobilizing resources to impact the team’s performance; Track your team’s success in achieving the goal

What They're Looking For.

Must Have

Minimum of two (2) years' management experience remote teammate management, Minimum of two (2) years' project management and/or project implementation experience required, Minimum of one (1) year experience demonstrating understanding of commercial insurance plans and benefits (PPO, HMO, POS, EPO, Indemnity) as well as government insurance programs (Medicare, Medicaid) required, Operational or client management experience across multiple sites required, Experience driving results for your book of business, Experience with KPIs, Experience mobilizing resources to impact team performance, Experience with root-cause analysis, Experience with flow charts, Experience tracking team success in achieving goals

Nice to Have

healthcare industry management experience preferred, Master's preferred
