Full Job Description

The LPN is responsible for delivering nursing service duties, in accordance with Healthcare Provider orders while providing oversight to the resident care and Medication Management Program in accordance with healthcare provider orders, state regulations and SRG policies. Our community setting is a relaxing change from a hectic hospital. Here at SRG you have a chance to get to know your patients; our residents count on the LPN to provide compassionate care. You will provide residents with nursing services in accordance with physician’s orders. The LPN is responsible for the administration of medications and treatments. Essential Duties & Responsibilities: * Work as part of the team within the Health Services Department * Participates in and completes resident’ assessments to identify resident care needs. * Develops and updates resident service plans which include person centered approaches and interventions to determine resident care level. * Follows up on residents’ health conditions, and /or changes of condition with healthcare providers, residents and responsible parties. * Communicates relevant resident information to healthcare providers, alerts community staff and responsible parties of care concerns and service plan changes. * Performs nursing services as directed and in accordance with the Nurses Practice Act, and state regulations. * Develops and maintains effective relationships and communication with residents, families, responsible parties and other healthcare providers. * Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care. * Assist with coordination of care for new move ins, residents on extended leave of absence and/or residents retu
