What You'll Do.
Deliver nursing services
Provide oversight to resident care
Provide oversight to Medication Management Program
Administer medications and treatments
Participate in resident assessments
Identify resident care needs
Develop resident service plans
Update resident service plans
Follow up on residents' health conditions
Follow up on changes of condition
Communicate resident information to healthcare providers
Alert community staff of care concerns
Alert responsible parties of care concerns
Perform nursing services
Maintain relationships with residents
Maintain relationships with families
Maintain relationships with responsible parties
Maintain relationships with healthcare providers
Review weekly visit notes
Ensure continuity of care
Coordinate care for new move ins
Coordinate care for residents on leave
Coordinate care for residents returning from alternate levels
Coordinate care for residents receiving services from new
Complete documentation via Yardi EHR
Initiate internal incident report
Initiate state reports
Participate in incident investigations
Coordinate incidental medical care
Coordinate incidental dental care
Coordinate incidental vision care
Coordinate incidental hearing care
Coordinate incidental podiatry care
Coordinate telehealth visits
Coordinate onsite lab diagnostic testing
Coordinate durable medical equipment
Complete Long Term Care insurance verification
Support Medication Management Program
Manage medication delivery services
Manage medication storage
Manage medication security
Collaborate with pharmacy partners
Assist with medication administration
Assist Director of Assisted Living with staffing
