What You'll Do.

**Location:** Avera Sacred Heart Hospital **Worker Type:** Regular **Work Shift:** Day Shift (United States of America) **Pay Range:** _The pay range for this position is listed below. Actual pay rate dependent upon experience._ $24.50 - $33.75 **Position Highlights** **HOURS:** Three 12hr day shifts (Monday-Friday); Split between Medication Reconciliation and Med Surg Unit ****3 shifts in Med Rec one week** ****2 shifts in Med Rec & 1 shift on Med Surg Unit the next week** **Student Loan Repayment:** This position may be eligible for Avera’s Student Loan Repayment Program, which helps you pay off your loans and save money. Work where you matter.** **A Brief Overview** Completes the home medication reconciliation process upon admission. The LPN will work collaboratively with patients, families, physician, pharmacists, clinics, home health and other agencies to ensure an accurate and complete record of medication is maintained. This position will also be scheduled nursing shifts for the Medical/Surgical department and will practice under the supervision of a registered nurse, advanced nurse practitioner, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members. **What you will do ** * Demonstrates ability to perform all essential job functions of an LPN, collects data and conducts a focused nursing assessment of the health status of a patient. * Obtains and assures complete and accurate medication history upon admission. * Assures medications are ordered upon admission; assists with medication lists on discharge as time permits. * Assists
