What They're Looking For.
Must Have
Active compact multi-state LPN/LVN license in the state where you currently reside, Active CA state LVN license, At least 5 years of clinical experience as a Licensed Practical Nurse
Nice to Have
Fluency in Spanish, Experience providing patient education and support to individuals managing chronic conditions
What You'll Do.
Provide virtual patient support across Cadence care programs
Support Medicare patients living with chronic conditions by reinforcing care plans
encouraging adherence to medications and preventive care
and providing education
patient-centered outreach and follow-through
Conduct structured patient check-ins
coaching and goal-setting
Care coordination to close gaps in care
Addressing barriers such as access to resources
or medication refills
Document patient interactions in the Cadence platform
Collaborate closely with the clinical team to escalate concerns
coordinate next steps
Conducting outbound telephonic patient outreach
Provide one-on-one coaching and support to patients managing chronic conditions
Help patients execute personalized care plans and achieve their goals by providing education and coaching
Conduct comprehensive assessments of patients' health status
and readiness to change
Support patient care delivery by performing assigned tasks in accordance with Cadence clinical policies and established protocols
under appropriate clinical supervision
Educate patients on disease management
and prevention strategies
Monitor patients' progress
adherence to treatment plans
and health outcomes through regular check-ins and remote monitoring tools
Documenting patient interactions
and follow-up plans in the Cadence platform
