What They're Looking For.
Must Have
Master’s Degree in Social Work, Counseling or related field, Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or have a MSW with the requirement to become licensed within 6 months of hire date, Basic Life Support (BLS) certification, 3 years acute care hospital experience, knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services
Nice to Have
Bilingual Spanish speaking, Additional related education and/or experience

Plan for safe discharge and continuity of care
Assess functional and psychological needs
Assess patient's home situation and resources
Formulate plan of intervention
Collaborate with healthcare team
Document all interventions
Function as liaison between hospital and community
Maintain knowledge of Medicare/Medicaid benefits
How You'll Work.
Team & Collaboration
Partners closely with patients, clinical teams, and community resources; Collaborates with all members of the healthcare team; Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team.
