What You’ll Be Doing: Serve as a compassionate liaison between residents, families, and physicians, collaborating with charge nurses for each shift to meet the needs of residents and the community Provide clinical support (Med Pass 12pm-2:00pm - 2nd floor, quarterly and PRN psychotropic reviews), supervision and assist with daily living activities (meals, bathing, grooming, mobility, etc.) Monitor resident health (RWO, HSE, skin checks) and maintain accurate care documentation (Incident reporting, alert charting, BM/ shower log), including medication chart audits and others as assigned Facilitate continued education with caregiving staff on a continuum Support a safe, respectful, and engaging environment for residents with memory impairments Qualifications: Graduate of an accredited Vocational Nursing program Active LVN/LPN license Preferred: experience in senior care or memory care settings Reliable transportation to meet attendance and punctuality standards Effective communicator in verbal and written English Active role: walking, bending, lifting (up to 50 lbs.)
