What They're Looking For.
Must Have
Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario, Minimum of 5 years nursing experience in a community setting, Minimum of 2 years in a management role, Experience working with palliative care patients, Experience with program development, implementation and evaluation, Valid G driver's license and reliable vehicle, Available for on-call rotation after hours and weekends
Nice to Have
Experience managing teams in a unionized environment, Experience working with care coordination

Lead an integrated palliative care team
Manage day-to-day coordination of services
Ensure continuity of care
Ensure seamless processes
Ensure optimal patient experience
Ensure optimal employee experience
Collaborate to assess care plans
Collaborate to plan care plans
Collaborate to coordinate care plans
Support patients to die in preferred place
Respect patient wishes for MAID
Oversee indirect care coordination activities
Monitor indirect care coordination activities
Support team to provide high quality care
Build trusting relationships
Understand patient goals
Build key internal stakeholder relationships
Build key external stakeholder relationships
Address quality issues
Address safety issues
Develop improvement plans
Participate in Quality-of-care Reviews
