What They're Looking For.
Must Have
Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario, Minimum of 5 years nursing experience in a community setting, Minimum of 2 years in a management role, Experience working with palliative care patients, Experience with program development, implementation and evaluation, Valid G driver's license and reliable vehicle, Available for on-call rotation after hours and weekends
Nice to Have
Experience managing teams in a unionized environment, Experience working with care coordination

We are currently seeking a dynamic and strategic Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as an Integrated Care Program Supervisor to lead a diverse, multidisciplinary team that will deliver high-quality, integrated palliative care in the Mississauga community and surrounding area. Working closely with health system partners, you will be an integral part of shaping the future of health care in Ontario. If you are a transformational leader, we want you on our team! This is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector where you’ll join a cultivate and high-performing team of Nurses, Personal Support Workers (PSWs), and Allied Health professionals, you’ll manage and foster a collective, innovative, and learning-focused environment. You’ll lead and support the implementation of integrated care programs focusing on optimizing service delivery through the use of data-driven insights.
