Full Job Description
## Accountabilities Receive, review, and process home health referrals accurately and in a timely manner, ensuring all patient and clinical information is correctly entered into the system. Perform basic diagnosis coding based on referral documentation and ensure data integrity within intake records. Coordinate patient intake workflows by applying structured communication approaches, including scripted welcome calls and standardized procedures. Collaborate with clinical intake teams to determine referral appropriateness and ensure proper routing of patient cases. Interface with external referral sources and internal teams to facilitate smooth communication and continuity of care. Support timely patient onboarding by ensuring all intake steps are completed efficiently and in compliance with organizational standards. Maintain accuracy, organization, and responsiveness in a high-volume, time-sensitive operational environment. Requirements: Homecare Homebase (HCHB) experience is required. Associate’s degree preferred. Minimum of 1 year of experience in home health or a related healthcare environment preferred. At least 1 year of experience in a customer service or patient-facing support role. Strong attention to detail with the ability to accurately process and manage clinical and administrative data. Excellent communication skills for interacting with referral sources, clinical teams, and patients. Ability to work effectively in a structured, fast-paced, and deadline-driven environment. Strong organizational and multitasking skills, particularly in managing high volumes of intake requests. Ability to work weekends (Saturday and Sunday coverage required) and choose between 5x8 or 4x10 schedules.
